COFMA has moved!

Our new clinic is located at 381 Kent St., Suite 201.

To open the door at the front entrance to the building for daytime appointments, enter *3705 on the intercom keypad.
For evening appointments, you will need to use the door buzzer to speak with reception to unlock the entrance.

 

New office policy 2024: Please arrive 10 minutes early to your appointment time.

This will allow you adequate time to remove outerwear, register at the front desk, and for our nurses to bring you to the exam room and perform their pre-assessment. We thank you in advance for your cooperation in this matter.
