Notice to Dr. Leclair’s Patients

**Information on Dr. Leclair’s upcoming move from COFMA to Brightside Medical Clinic**

The patient enrolment form for the new clinic is now available and can be accessed by clicking here

This is the same form you would have signed when joining my current practice. I will have the same expectations of my rostered patients after I move to the new location including:

                   – Please pre-book all appointments (either by phone or online). Do not show up at the clinic expecting to be seen.

                   –  If you need to cancel an appointment, please do so within 24 hours if possible.  This allows us to book someone else in your place.

                   – Please do not visit walk-in clinics. We strive to provide timely access to patients so that this is not necessary.

As I mentioned in my previous communication, should you wish to remain a patient of my practice and follow me to the new location, you will need to complete the Ministry of Health Patient Enrolment Form and submit it by April 1, 2023. There are multiple ways that you can submit the form:

          (1)    The form is available for download by clicking here.  You can download and print the form then email a signed copy (page 3 only) to brightsideroster@gmail.com.

          (2)   Download and print the roster form by clicking here and mail it (page 3 only) to either clinic (see addresses below). You can also drop it off in person.

          (3)   Copies of the form will be available at both COFMA and Brightside clinics and can be signed on site. Please date the form no earlier than April 1, 2023.
